Volunteer/Volunteer Family of the Year

The OFATV Volunteer Awards and Family Volunteer Awards will be presented at the OFATV Annual General Meeting on the basis of nominations received from the general membership. The nomination must be on the official form (see below) and include the name and address of the OFATV member making the nomination, name and address of the nominated person/family and a brief description of why the nominee or nominee family is deserving of the award.
